Abstract
Revascularization therapies are essential for efficient treatment of acute coronary syndrome. Yet, besides rescuing the affected myocardium from excessive damage, reperfusion also inflicts additional injury. In this study, we assessed levels of reactive oxygen species (ROS), the main culprits of reperfusion injury, in myocardium of patients undergoing revascularization after acute coronary syndrome. We found that revascularization by coronary artery bypass grafting after non-ST elevation acute coronary syndrome is associated with significant increase in ROS levels in patients without developed coronary artery collaterals, but patients with collateralization exhibit no such effect of reperfusion. This demonstrates protective effect of coronary artery collateralization and re-iterates the vulnerability of patients without collaterals, making them most appropriate candidates for cardioprotective interventions targeting the reperfusion injury.
